Clinical Community Health Services

Raukura Hauora O Tainui offers a range of community health services providing a selection of quality healthcare programmes for all our individuals or whaanau at FREE or low and affordable costs.

We have a strong workforce of qualified and trained staff who are committed to support you in making better health choices to improve your own health and wellbeing in a culturally and supportive manner.  Our services are offered in both Taamaki and Waikato regions. 

Waikato Services

Mobile Chronic Care Management: This service is to help support and manage patients with chronic conditions. These conditions can range from diabetes, heart disease, mental health, asthma etc. Our well-trained nurses can provide support, education and help with managing chronic conditions to enable our patients to be independent in managing their health.

Maternity Support / Hapu Maamaa Support: This service is to help support our hapu mama during their pregnancy. Our trained coordinator is able to support with education, access to maternity services and Hapu Maamaa programmes.

Tamariki Ora: This service is the Well Child/Tamariki Ora national service. Our highly trained and skilled nurses help to support our tamariki from the ages 6 weeks to 5 years. They will support parents or carers with their child reaching their milestones of development through these very special early years from crawling, speech to oral health.

Outreach Immunisations Service: Our highly skilled nurses are well trained in administering immunisations in a range of environments including koohanga, early childhood centres, homes and community settings. This service is only available by specific referral via medical clinics.

Rheumatic Fever Prevention: Throughout all of services and our clinics we offer free education and throat swabbing for any child or youth to the age of 18 and their family/household members. All our nurses working in the community and clinics can offer this service.

Primary Mental Health: This service is provided across the Waikato. Our skilled coordinator works alongside our medical clinics to receive referrals from our Doctors to provide a range of counselling services and referrals to psychiatrists if required. This includes adolescent and adults.

Community Health Workers: This service provides advocacy, support and education for whaanau who have or are at risk of type 2 diabetes, cardiovascular disease and respiratory illnesses. Our qualified staff also provide specialist dietary and fitness advice for tamariki, rangatahi, pakeke and koroua.  

Whanau Ora Services: Our Whanau Ora practitioners provide social, financial, housing and employment support for whanau. Our specialised staff use comprehensive planning and assessment tools to enable whanau to understand their situation and take the necessary steps to achieve their dreams and goals.

Disability Support Services: Our Maaori Disability Service is aimed at improving the quality of life and independence of people with disabilities. Our Disability Support coordinator provides advice and advocacy services to people with long-term disabilities. In addition to this, the coordinator facilitates access to disability support services including; Needs Assessment Service Coordination (NASC) and referred services (aged residential care, home support).

All of these services are FREE based on eligibility for public health funding.

Taamaki Services

Mokopuna Ora - Well Child/Tamariki Ora and Outreach Immunisation Services

The Mokopuna Ora team consists of caring, professional nurses and support workers who provide care and support for tamariki and whaanau in the South Auckland area. 

They provide: Well Child core checks for tamariki aged 0-5; Before School checks; advocacy and support, and referrals to other health and social services. 

Rongoaa Maaori Traditional Healing

The Rongoaa Maaori service provides Maaori medicine and healing through mirimiri, karakia and whitiwhiti koorero to people in both the Taamaki and Waikato regions. The team works with both men and women of all ages and their whaanau in Counties Manukau and Waikato DHB areas.

Oranga Ki Tua - Whanau Living with Long-term Conditions

The Oranga Ki Tua team delivers a whaanau ora outcomes-focussed service to whaanau who are living with long-term conditions such as diabetes, gout, asthma, and cardiovascular disease. 

The team works with:

• Adults aged 25-64
• Hard-to-reach adults with long-term conditions, in particular adults that identify as Maaori
• Living in Manurewa and Manukau
• Assessed as being vulnerable or having high or complex health needs
• Eligible to receive publicly funded healthcare in NZ

Referrals can come through self-referrals, other health or social service providers, primary care or locality network lead hubs.

Whaanau Oranga - healthy lifestyles

We support whaanau to take control and responsibility of their hauora. Whaanau Oranga is focused on lifting the quality of life of our whaanau members; we want you to thrive!  

The team can coordinate tailored care plans based on the whaanau's needs which could include safety and support; enrolment into primary care; quitting smoking; accessing dental care; suicide/self-harm; drug and alcohol addiction support; financial services advice, literacy and more. 

Whaanau Oranga works with:

• Adults aged 25-64
• Hard-to-reach adults with social needs, in particular, those who identify as Maaori
• Vulnerable people or those with high or complex social and health needs
• Living in Manurewa and Manukau 
• Eligible to receive publicly funded healthcare in NZ

Referrals can come from self-referrals, other health care service providers, Police, other social service providers.

Public Health Services

Raukura offers a range of public health services including leadership and promotion in the following areas: Tihei Mauri ora - reducing tobcacco use; Kia Maarie - reducing alcohol and other drug use; Whai Ora - improving nutrition and exercise; infrastructure - leadership, innovation and Whaanaungatanga (workforce development).

We work with whaanau, hapu, iwi, and Maori communities in Taamaki Makaurau / Auckland region.

Suicide Prevention Service

Kia Piki te Ora is an all-age suicide prevention service that aims to promote mental health and wellbeing for Maori, to increase the safe reporting of suicide by the media and to contribute to improved mental health services for Maori.

The team's work means they work directly with Maori and their whaanau in South Auckland/Counties Manukau as well as with hapuu, iwi and hapori to educate about suicide awareness and prevention. They also work with key stakeholders at local, regional and national levels.
